It is necessary to provide accountability, structure and support to an individual leaving treatment. Our recovery advocates (case managers) follow through with individuals to help them and the treatment center meet their goals.
Recovery Advocates / Case Managers:
-Develop individualized treatment plans in collaboration with client addressing short term and long-term goals.
-Provide on-going support to assess progress and ensure treatment plan outcomes are met or changed as needed.
-Monitor drug and alcohol testing orders and results
-Meet weekly with clients via phone or video chat to address barriers for clients to obtain and maintain housing.
-Conduct crisis and suicide risk assessment, provide appropriate interventions as needed, and consult with LCSW supervisor, complete critical incident reporting with 24 hours
-Provide crisis intervention services focused on enhancing the client’s ability to independently problem solve, use effective coping skills, and manage and self-coordinate own care.
-Support the client’s move from a high intensity level of service to a more independent, recovery focused level of service as appropriate.
-Complete progress notes on every contact with client or collateral contact by the next business day.
